For prospective patients:
Available at request, and are per 90 minute session. Fees are due at the time of the appointment. Currently I am private pay only, but can provide a form for you to seek reimbursement from your insurance company. I plan on getting on the panel for some insurers in the future. I do not take Medicare.
How to arrange therapy:
Once we make contact, I will request a short phone consultation and that you complete a form of your history for me to review. If we make the decision to work together, we will then find a time suitable for us both to meet.
Cancelation/late arrival policy:
I require 24 hours notice for a cancelation. If you need to cancel less than 24 hours before our appointment, you will be charged for the full appointment. Insurance companies do not reimburse for missed visits. Punctuality is important in the therapy process. If you know you will be arriving late, please call and let me know as soon as possible. Once you arrive, we will meet for the remainder of the time allotted in our appointment, and you will be charged the full amount. If I don't hear from you within 15 minutes after the appointment start time, I will assume you are missing the appointment. If you arrive greater than 5 minutes late to more than 3 appointments, we will discuss this issue and it may result in termination of our work together. If you have one no-show without notification, we will discuss this issue and it may result in termination of our work together.
I have limited time per week to provide therapy in this practice. If we discuss the idea of working together, and I am full, you will be placed on a waitlist until a spot becomes available.
